Q. What are your location hours?
A. We are opened from 8am to 5pm Monday through Friday.

Q. Do I need a referral?
A. Most patients do not require a referral. If your insurance is an HMO/managed care plan and you are required to obtain a referral from your primary care physician, you must obtain before you can be seen in our office.

Q. What do I need to bring to my appointment?
A. Please bring the following:

  • Insurance Card
  • Copies of imaging MRI, CT, X-ray
  • Notes from previous treatment or providers for current issues.
  • Registration paperwork- Download forms

Q. Do you accept my insurance?
A. Patients are responsible for confirming with their insurance company that our providers are in-network with their plan.

Q. How can I get a copy of my medical records?
A. Please complete our medical records request form. Medical Records request are processed as quickly as possible but can often take 1-2weeks to complete. Medical records cannot be released without written consent from the patient or guardian.

Q. When is payment due and what payment methods do you accept?
A. Insurance plans require you to pay a co-payment, co-insurance, and/or a deductible, you will need to pay at the time of your visit. For your convenience we accept cash, checks, Visa, Mastercard, and Discover.
